This 45 year old executive in a multinational company had headaches and visual field deficits of 5 months duration. The MRI brain showed a large meningioma arising from diaphragm sellae. This was excised through a small supraorbital craniotomy completely. It was very vascular and was draped by Anterior cerebral arteries with its branches and optic nerves at periphery. The visual deficits improved completely, but patient had diabetes insipidus.
